Noticing a strong or unusual smell in your urine—without any pain—can be confusing and sometimes worrying. While urine naturally has a mild scent, certain changes can make it smell unusually strong, sweet, fishy, or musty. Some causes are harmless, but others may point to underlying health issues that require medical attention.

Understanding why urine changes smell is important for maintaining good urinary and overall health. Below are the most common causes and when to seek help.

1. Diet

What you eat directly influences how your urine smells. Foods like asparagus, garlic, onions, fish, broccoli, cabbage, eggs, coffee, alcohol, and certain spices contain compounds that break down into substances released through urine. These compounds can produce strong odours shortly after eating and may last for hours.
If you suspect a certain food, try removing it for a few days to see if the smell improves.

2. Dehydration

When you don’t drink enough water, urine becomes darker and more concentrated with waste products, making its natural scent stronger. Signs of dehydration include dark yellow urine, dry mouth, fatigue, headaches, and increased thirst.
Increasing your water intake often restores urine to a lighter colour and reduces the smell.

3. Urinary Tract Infection (UTI)

A UTI can cause foul-smelling urine even before other symptoms appear. Bacteria such as E. coli release strong-smelling chemicals as they multiply in the urinary tract. Other symptoms may include cloudy urine, frequent urination, urgency, or a burning sensation. Untreated UTIs can spread to the kidneys and lead to serious complications, so early treatment is important.

4. Sexually Transmitted Infections (STIs)

Some STIs, including chlamydia and trichomoniasis, can change the smell of urine. The odour often comes from abnormal discharge mixing with urine.
Additional symptoms may include itching, soreness, or painful urination.
Prompt testing and treatment help prevent long-term complications.

5. Medications and Vitamins

Certain antibiotics, diabetes medications, arthritis drugs, and supplements can temporarily change urine odour.
Vitamins such as B-complex (especially thiamine and choline) are well-known for producing a strong or unusual smell after the body excretes excess amounts.

6. Diabetes

Uncontrolled diabetes often causes sweet or fruity-smelling urine due to high levels of glucose or ketones.
In severe cases, a strong acetone-like smell can indicate diabetic ketoacidosis, a medical emergency that requires immediate care. Monitoring changes in urine can help detect early warning signs of diabetes complications.

7. Liver or Kidney Problems

The liver and kidneys filter toxins from the bloodstream. When these organs are not functioning properly, waste products build up and alter the smell, colour, or consistency of urine. Conditions such as kidney disease, kidney stones, or metabolic disorders may cause strong-smelling urine, along with symptoms like swelling, fatigue, abdominal pain, or changes in urine flow.
Early diagnosis can prevent long-term organ damage.

Causes More Common in Females

People assigned female at birth may experience foul-smelling urine due to hormonal changes, pregnancy, or bacterial imbalances.
Conditions such as bacterial vaginosis can produce fishy-smelling discharge that mixes with urine.
Pregnancy-related hormonal changes, prenatal vitamins, and increased UTI risk can also affect urine odour.
During ovulation, changes in vaginal mucus can make urine look cloudy, although this typically doesn’t cause a foul smell.

When to See a Doctor

Seek medical evaluation if foul-smelling urine is accompanied by any of the following:
Cloudy urine
Burning sensation
Frequent or urgent urination
Blood in the urine
Fever or chills
Lower back or abdominal pain
Fatigue or confusion

These symptoms may indicate infections or organ issues that need prompt treatment.
